Agentic AI Security: A CISO's Guide to Hybrid IT/OT

As AI agents begin to take actions across systems, they must be governed like privileged users, with strict access control, human approval, and real-time monitoring.

What is Agentic AI Security

In simple terms, agentic AI security is about controlling actions, not just outputs.

Enterprise spending on agentic AI is rising fast, and that matters for one reason. These systems do more than generate text. They take actions inside real workflows.

Agentic AI is software that receives an objective, decides on steps, uses tools, and keeps working until it reaches a result or hits a control boundary. A standard model answers a prompt. An agent can query a system, call an API, open a ticket, summarize findings, and choose the next step without waiting for a user after every action.

For a CISO, the label matters less than the operating model. If the system can act with persistence, use enterprise tools, and influence production decisions, treat it as an operational actor.

What makes a system agentic

The defining traits are straightforward:

  • Goal-driven behavior. The system is assigned an outcome, not just a question.
  • Tool use. It can call APIs, query databases, trigger scripts, or interact with business and industrial software.
  • Planning. It can break work into steps and decide what to do next.
  • Memory or state. It can retain context across tasks or sessions.
  • Adaptation. It can change course when conditions change or a step fails.

Those capabilities create business value. They also create a different security problem. You are no longer containing inaccurate output alone. You are controlling machine-initiated behavior across connected systems, including systems that were never designed for dynamic autonomy.

That distinction is easy to miss in hybrid IT and OT environments. An agent reviewing maintenance records, vendor documentation, historian data, or incident tickets may never directly touch a controller, yet it can still shape operator decisions, service timing, and escalation paths. In air-gapped or intermittently connected sites, the same agent may run with local models, delayed policy updates, and weaker telemetry, which makes oversight harder and mistakes slower to detect.

Why deployment is accelerating

Adoption is accelerating because agents promise labor savings and faster execution. Do not read that as a buying signal. Read it as a governance deadline.

An agent does not need human judgment to create operational risk. It only needs access, instructions, and enough freedom to act.

A practical way to explain this to leadership is simple. A chatbot produces content. An agent participates in operations. That is the line your policies, approvals, logging, and segmentation model must reflect.

How Agentic AI Changes the Security Model

The key change is that AI systems are no longer passive—they act.

Agentic AI isn’t another analytics tool you can bolt onto the SOC and govern later. It can observe, decide, call tools, trigger workflows, and act across systems with limited or no human review. In a hybrid IT/OT environment, that means the blast radius is no longer confined to a dashboard, a ticket queue, or a cloud workload. It can reach privileged sessions, maintenance workflows, industrial operations, and vendor access paths.

Most advice on agentic AI security still assumes a pure IT estate. That’s inadequate. A manufacturing plant, telecom core, or regulated remote maintenance environment has very different failure modes. Why does this change the CISO agenda?

Autonomous AI changes the unit of risk. Traditional tools mostly assist analysts within a defined workflow. Agentic systems can observe, select, invoke tools, and continue acting toward an objective across multiple systems. That means the security question is no longer just whether the model is accurate. The question is what the agent is allowed to do when it is wrong, manipulated, overconfident, or operating in an incomplete context.

A triage assistant that summarizes alerts is still software support. An agent that correlates telemetry, opens tickets, queries internal systems, triggers scripts, or initiates containment resembles a junior operator with machine speed and inconsistent judgment.

CISOs need to classify that correctly.

What Risks Exist in Agentic AI Systems

The market excitement is understandable. The security exposure is usually underestimated.

According to Martin Fowler’s analysis of agentic AI security, the average cost of AI agent-related data breaches reached $4.7M in 2026, 1 in 8 corporate data breaches is now linked to AI agent activity, and 34% of deployed agents have been affected by prompt injection attacks. That should end any fantasy that agents are just another interface problem.

Prompt injection is only the start

Organizations are familiar with the term "prompt injection," but many still think of it as a chatbot issue. In agentic systems, it becomes an execution issue.

An attacker doesn’t need to breach your infrastructure directly. They can hide malicious instructions in content that the agent is allowed to read. That could be a ticket comment, a document, a knowledge base article, or data returned from an external service. If the agent treats that content as guidance, it may take action you never intended.

Three related risks matter most in practice:

  • Indirect prompt injection. The malicious instruction arrives through a source the agent trusts enough to read.
  • Tool misuse. The agent uses a legitimate API or system function for the wrong purpose.
  • Goal hijacking. The agent starts with an approved objective, then gets steered into an unsafe one.

Memory and orchestration create durable risk

Agents don’t just read and act. Many keep state. That means bad input can persist.

A poisoned memory store can distort future decisions long after the original payload disappears. A corrupted internal note, a false dependency, or a manipulated instruction can persist across sessions and later affect unrelated workflows. In a multi-agent design, a compromised agent can also pass tainted context to another agent.

That’s why the risk surface is larger than the model itself. You have to secure:

The core design flaw is architectural

At the heart of agentic AI security is an uncomfortable fact. Large language models (LLMs) cannot reliably distinguish between instructions and untrusted data in adversarial scenarios. If the agent reads it, the model may treat it as something to act on.

Security teams should stop asking whether an agent is smart enough to avoid manipulation. Ask whether the architecture assumes manipulation will happen.

That mindset leads to better controls. It pushes you toward constrained access to tools, isolated execution, approval gates, and runtime enforcement rather than blind trust in model behavior.

How to Secure Agentic AI Systems

Most organizations start in the wrong place. They focus on model safety and forget system control. For CISOs, agentic AI security is an identity, authorization, and runtime governance problem.

The first principle is simple. Treat every agent as a non-human identity. In practice, this often maps to service accounts or application identities, but the control model should be equivalent. Give it an owner, a purpose, a bounded scope, and explicit entitlements. If your IAM, PAM, and change-control processes don’t recognize agents as identities, your governance model is already broken.

Start with least privilege and hard boundaries

Agents should never inherit broad access just because they operate inside a trusted workflow. Give each one access only to the tools, data sources, and APIs required for a single defined function.

That means:

  • Separate read from write. Many agents only need visibility, not execution rights.
  • Limit the tool scope. Allow specific functions, not entire platforms.
  • Constrain data reach. Don’t let one agent traverse unrelated business or operational domains.
  • Time-box expanded access. If the agent needs more privilege, make it temporary and task-bound.

Many programs fail due to a common issue. Teams grant broad backend access to “make the demo work,” then try to bolt controls on later.

Move beyond static roles

Static RBAC is not enough for autonomous systems. Palo Alto Networks’ guidance on agentic AI security is right on this point. Securing multi-agent systems requires moving beyond static RBAC to behavioral authorization and continuous validation, and runtime guardrails must enforce system-level boundaries so that even tampered agents can’t exceed intended scope.

That means authorization should answer more than “Does this identity have permission?” It should also ask:

Is this action normal for this agent? Detects drift and misuse
Is the timing expected? Flags suspicious triggers
Is the target system in scope? Prevents lateral spread
Is the sequence of actions reasonable? Catches chained abuse
Should a human approve this step? Adds control at critical moments

Operational advice: If your only control is a role assignment, the agent already has too much freedom.

A strong control stack should include:

  • Agent identity attestation tied to a clear owner and purpose
  • Policy-based approval gates for sensitive actions
  • Tool allowslists and explicit deny rules
  • Continuous monitoring of behavior across sessions
  • Immutable logs that capture what the agent did and why
  • Kill switches and graceful fallback to lower autonomy when risk spikes

Why Privileged Access Matters

If an AI agent can act across systems, it must be treated and controlled like a privileged user.

Agentic AI systems should not be treated as just another software layer. They operate as non-human identities within your environment.

Each agent has access to systems, tools, and data. It makes decisions, triggers actions, and interacts with workflows that would traditionally require a human operator. From a security perspective, that makes an agent functionally equivalent to an identity.

Once you recognize an agent as an identity, the next step is unavoidable.
It becomes a privileged identity.

And once an agent operates with privilege, the next requirement follows naturally.

Privileged actions require control.

An agent that can query internal systems, trigger workflows, or influence maintenance and vendor access paths is operating with elevated authority. In hybrid IT and OT environments, that authority can directly impact production workflows, remote sessions, and operational integrity.

This is where many organizations get it wrong. They focus on model behavior, but ignore execution control.

This is not just an AI problem. It is an access control problem.

If an agent can act, it must be governed with the same rigor as a privileged human user.

In critical environments, not all actions should be fully autonomous.

Actions such as accessing sensitive systems, triggering operational changes, or interacting with vendor environments are not just technical steps. They are decision points with real impact.

That means:

  • Explicit authorization boundaries for every tool and system
  • Human approval gates for high-impact actions
  • Continuous monitoring of agent behavior and sessions
